The House of Care Toolkit:<\/p>\n<ul>\n<li>Provides a framework to bring together all the relevant national guidance, published evidence, local case studies and information for patients and their carers<\/li>\n<li>Includes information on what tools and resources are required to achieve person-centred coordinated care and how these can be effectively commissioned<\/li>\n<li>Contains resources that are arranged into the key components of the House with and details where to find additional information<\/li>\n<\/ul>\n<hr \/>\n<p><strong>Commissioning sexual health, reproductive health and HIV services &#8211;\u00a0HM Government<\/strong><br \/>\nThis <a href=\"https:\/\/www.gov.uk\/government\/publications\/commissioning-sexual-health-reproductive-health-and-hiv-services\" target=\"_blank\">guide <\/a>looks at how to pull the whole commissioning system together, focusing on:<\/p>\n<ul>\n<li>interfaces in commissioning responsibility, detailing the areas where more than 1 commissioning organisation is responsible for different elements of care that an individual may need. Includes how commissioning bodies need to work together to ensure that the individual experiences seamless delivery of services to meet their needs.<\/li>\n<li>addressing the wider determinants of health, illustrating examples of how local areas are taking a wider view to address an area of need. Also to build and develop capacity in its leadership team and staff.<\/p>\n<hr \/>\n<p><strong>North East Lincolnshire CCG: Organisational Development Planning \u2013 NHS Commissioning Assembly Video<\/strong><br \/>\nYoutube video looking at how \u00a0North East Lincolnshire CCG have used <a href=\"https:\/\/www.youtube.com\/watch?v=VXPxUetPLgg\" target=\"_blank\">organisational development <\/a>to develop effective partnerships and decision making bothin within the organisation and with external partners.<\/p>\n<hr \/>\n<p><strong>Eastbourne, Hailsham &amp; Seaford CCG and Hastings &amp; Rother CCG: Organisational Development Planning \u2013 NHS Commissioning Assembly Video<\/strong><br \/>\nYoutube video looking at how Eastbourne, Hailsham &amp; Seaford CCG and Hastings &amp; Rother CCG have used <a href=\"https:\/\/www.youtube.com\/watch?v=gtn5Oh2qIs0\" target=\"_blank\">organisational development<\/a> to bring two CCGs together to tackle significant organisational and financial challenges.
