href=\"https:\/\/www.rcplondon.ac.uk\/guidelines-policy\/mortality-toolkit\">The National Mortality Case Record Review toolkit<\/a>.\u00a0\u00a0 This toolkit has been developed to support trusts in implementing a standardised way of reviewing the case records of adults who have died in acute hospitals across England and Scotland.<\/p>\n<hr \/>\n<p><strong>Reducing emergency admissions<\/strong><br \/>\nThe Public Accounts Committee has published <a href=\"https:\/\/www.parliament.uk\/business\/committees\/committees-a-z\/commons-select\/public-accounts-committee\/news-parliament-2017\/reducing-emergency-admissions-report-published-17-19\/\">Reducing emergency admissions<\/a>.\u00a0 The Committee finds it lamentable that nearly 1.5 million people could have avoided emergency admissions in 2016\u201317 if hospitals, GPs, community services and social care had worked together more effectively.\u00a0 They say it is frustrating that NHS England and partners are making some progress in reducing the impact of emergency admissions for patients and hospitals when they do happen, but no impact on reducing the numbers of admissions that could have been avoided.<\/p>\n<hr \/>\n<p><strong>Standards for children in emergency care settings<\/strong><br \/>\nThe Intercollegiate Committee for Standards for Children and Young People in Emergency Care Settings has published <a href=\"https:\/\/www.rcpch.ac.uk\/resources\/facing-future-standards-children-young-people-emergency-care-settings\">Facing the future: standards for children in emergency care settings<\/a>.\u00a0 These seventy standards aim to ensure that urgent and emergency care is fully integrated to ensure children are seen by the right people, at the right place and in the right setting<\/p>\n<hr \/>\n<p><strong>Integrated care: organisations, partnerships and systems<\/strong><br \/>\nThe Health and Social Care Committee has published <a href=\"https:\/\/www.parliament.uk\/business\/committees\/committees-a-z\/commons-select\/health-and-social-care-committee\/news\/integrated-care-report-published-17-19\/\">Integrated care: organisations, partnerships and systems<\/a>.\u00a0 The Committee say the Government and the NHS must improve how they communicate NHS reforms to the public in their report on integrated care.\u00a0 The Committee recognises the potential benefits of further integration and calls on the Government to bring forward legislation to remove legal barriers imposed by the Health and Social Care Act 2012.\u00a0 The Committee says the Government must recognise the importance of adequate transformation and capital funding to effective service change.<\/p>\n<hr \/>\n<p><strong>Innovative models of general practice<\/strong><br \/>\nThe King\u2019s Fund has published <a href=\"https:\/\/www.kingsfund.org.uk\/publications\/innovative-models-general-practice\">Innovative models of general practice<\/a>.\u00a0 This report looks at innovative models of general practice from the UK and other countries and identifies key design features that will be important in designing effective GP services in the future.\u00a0 It makes recommendations for general practice, system leaders and commissioners, and national policymakers.<\/p>\n<hr \/>\n<p><strong>General practice forward view: BMA analysis<\/strong><br \/>\nThe British Medical Association has published <a href=\"https:\/\/www.bma.org.uk\/advice\/employment\/gp-practices\/general-practice-forward-view\/gpfv-two-years-on\">The general practice forward view: two years on<\/a>.\u00a0 This report evaluates how the implementation and delivery of the GPFV has progressed so far, and seeks to assess whether it is on track to deliver on its commitments.<\/p>\n<hr \/>\n<p><strong>Breaking down barriers to better health and care<\/strong><br \/>\nNHS England has published <a href=\"https:\/\/www.england.nhs.uk\/publication\/breaking-down-barriers-to-better-health-and-care\/\">Breaking down barriers to better health and care<\/a>.\u00a0 This guide summarises how NHS organisations and local councils in England are moving from fragmented services to local partnerships and integrated care systems in order to meet the health needs of the population.<\/p>\n<hr \/>\n<p><strong>Adult social care funding<\/strong><br \/>\nCommissioned by the Department of Health and Social Care, <a href=\"https:\/\/www.gov.uk\/government\/news\/new-research-into-funding-for-adult-social-care-published\">The review of adult social care relative needs formulae<\/a> has been published.\u00a0 This document proposes a new formula to be used with the Local Government Finance Settlement.\u00a0 The research includes separate formulae for dividing budgets for 4 specific aspects of the Care Act.\u00a0 These formulae were commissioned in 2012 and may need to be revised to reflect the outcome of the forthcoming green paper on adult social care<\/p>\n<hr \/>\n<p><strong>Patient experience improvement framework<\/strong><br \/>\nNHS Improvement has published <a href=\"https:\/\/improvement.nhs.uk\/resources\/patient-experience-improvement-framework\/\">Patient experience improvement framework<\/a>.\u00a0 This framework is aimed at supporting healthcare providers with an increased focus on empowering patients to fully participate in decisions about their care and treatment.\u00a0 It enables trusts to carry out an organisational diagnostic to establish how far patient experience is embedded in its leadership, culture and its operational processes.<\/p>\n<hr \/>\n<p><strong>Quality checkers toolkits<\/strong><br \/>\nNHS England has published toolkits for <a href=\"https:\/\/www.england.nhs.uk\/publication\/nhs-quality-checkers-toolkits\/\">NHS quality checking<\/a> where people with a learning disability, autism or both are employed to help look at the quality of the NHS services they use and make suggestions for how they can be improved.<\/p>\n<hr \/>\n<p><strong>Gross negligence manslaughter in healthcare<\/strong><br \/>\nThe Department of Health and Social Care has published <a href=\"https:\/\/www.gov.uk\/government\/publications\/williams-review-into-gross-negligence-manslaughter-in-healthcare\">Gross negligence manslaughter in healthcare<\/a>.\u00a0 This report from Professor Sir Norman Williams looks at gross negligence manslaughter in healthcare and makes recommendations to support a more just and learning culture in the healthcare system.\u00a0 It covers the process for investigating gross negligence manslaughter; reflective practice of healthcare professionals; and the regulation of healthcare professionals.<\/p>\n<hr \/>\n<p><strong>Clinical correspondence<\/strong><br \/>\nThe Public Accounts Committee has published <a href=\"https:\/\/www.parliament.uk\/business\/committees\/committees-a-z\/commons-select\/public-accounts-committee\/news-parliament-2017\/nhs-correspondence-handling-report-published-17-19\/\">Clinical correspondence handling in the NHS<\/a>.\u00a0 The Committee found that NHS England was too slow to tackle misdirected correspondence. \u00a0NHS England is still assessing nearly 2,000 cases to determine whether there has been harm to patients and has so far identified two incidents where expert consultant review has concluded that patient harm cannot be ruled out.<\/p>\n<hr \/>\n<p><strong>Walking briskly app<\/strong><br \/>\nPublic Health England and the Royal College of GPs are encouraging adults to focus on <a href=\"https:\/\/www.gov.uk\/government\/news\/focus-on-brisk-walking-not-just-10000-steps-say-health-experts\">walking briskly rather than just counting steps to improve their health<\/a>.\u00a0 To help adults do this, PHE\u2019s \u2018Active 10\u2019 app has been created; the app combines intensity and time, rather than just distance.<\/p>\n<hr \/>\n<p><strong>NICE
