As contracts expire, commissioners need to consider what options are best for patients. Results indicate that WiCs have significantly reduced attendances at hospital\u00a0Emergency Departments in places close by, but suggest that only between 10-20% of patients\u00a0seen at hospital-based WiCs and between 5-10% patients seen at other WiCs were diverted\u00a0from the more costly Accident and Emergency Departments\u00a0at hospitals.<\/p>\n<hr \/>\n<p><strong>Potentially Avoidable Hospitalisations in England \u2013 European Collaboration for Healthcare Optimization\u00a0<\/strong><br \/>\nThe <a href=\"http:\/\/echo-health.eu\/wp-content\/uploads\/2014\/11\/report_PAH_ENG_final_owen_logos_4.pdf\" target=\"_blank\">study <\/a>of systematic variations in Potentially Avoidable Hospitalizations (PAH) offers a critical view on how healthcare organizations provide care to patients with chronic conditions. In particular, it signals how effectively they are managed in the ambulatory setting. \u00a0This report analyses the magnitude and the variation in unplanned\u00a0hospitalisations from six chronic conditions highly sensitive to ambulatory\u00a0care: angina, adult asthma, congestive heart failure (CHF), chronic obstructive\u00a0pulmonary disease (COPD), dehydration and short-term complications of\u00a0diabetes; as well as an additional indicator aggregating admissions for all 6\u00a0conditions (All PAH).<\/p>\n<hr \/>\n<p><strong>Better Training Better Care Evaluation Reports \u2013 Health Education England\u00a0<\/strong><br \/>\nHealth Education England published the <a href=\"http:\/\/hee.nhs.uk\/wp-content\/blogs.dir\/321\/files\/2012\/09\/Matrix-Knowledge-Better-Training-Better-Care.pdf\" target=\"_blank\">evaluation report <\/a>to the Better Training Better Care pilot projects and the evaluation report to the national elements on 16 January 2015. Both reports set out what was achieved, the benefits that were realised and opportunities that have arisen, as well as the challenges and lessons learnt.<\/p>\n<hr \/>\n<p><strong>Using case finding and risk stratification: A key service component for personalised care and support planning \u2013 NHS England<\/strong><br \/>\nThis <a href=\"http:\/\/www.england.nhs.uk\/wp-content\/uploads\/2015\/01\/2015-01-20-CFRS-v0.14-FINAL.pdf\" target=\"_blank\">handbook<\/a> is one of three service components and provides some\u00a0valuable practitioner insights into case finding and risk stratification to\u00a0support personalised care and support planning \u2013 both of which are key\u00a0elements of the vision and outcomes in the NHS Five Year Forward\u00a0View. The Document will be uploaded onto the NHS England Website\u00a0and therefore will be available for all to access.<\/p>\n<hr \/>\n<p><strong>The use of outcome and process indicators to incentivize integrated care for frail older people: a case study of primary care services in Sweden \u2013 International Journal of Integrated Care\u00a0<\/strong><br \/>\nAims to analyse limitations in the use of outcome and process indicators to incentivize <a href=\"http:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC4272240\/pdf\/IJIC-14-2014038.pdf\" target=\"_blank\">integrated care for elderly patients with significant health care needs in the context of primary care.<\/a> Finds both the ranking exercise and regression models revealed important problems related to risk-adjustment, attribution, randomness and measurement fixation when using indicators in P4P schemes and for external accountability purposes.
