This report illustrates that allied health professionals are having a significant positive impact on the public\u2019s health and with further support this could make a real difference to the health and wellbeing of the population.<\/span><\/p>\n<hr \/>\n<p><strong>Toolkit for general practice in supporting older people with frailty and achieving the requirements of the Unplanned Admissions Enhanced Service (2014) \u2013 NHS England<\/strong><br \/>\nThe development of <a href=\"http:\/\/www.nhsiq.nhs.uk\/media\/2630779\/toolkit_for_general_practice_in_supporting_older_people.pdf\" target=\"_blank\">this toolkit<\/a> is a collaboration between New Devon CCG, NHS Kernow and the NHS England Devon, Cornwall and Isles of Scilly Local Area Team. Following masterclasses held across Devon and Cornwall for GPs and practice staff in May 2014, there were requests for a common approach to case finding, assessment, care planning and case management of frail older people. The GP2GP team produce a monthly <a href=\"http:\/\/systems.hscic.gov.uk\/gp2gp\/statisticsandreports\">CCG GP2GP utilisation report<\/a> showing all practices within England (live and not live with GP2GP) and detailed information to enable CCGs (and where designated, CSUs), to fully understand the utilisation of GP2GP within each region. Guidance and frequently asked questions are included in the report to help users better understand common queries, possible issues and scenarios. This report includes full details of each practice&#8217;s GP2GP requesting, sending and integration activity which should help identify potential barriers to using GP2GP.
