Nigel Edwards<br \/>\nSustainability and Transformation Plans (STPs) \u2013 the <a href=\"http:\/\/www.nuffieldtrust.org.uk\/publications\/sustainability-and-transformation-plans-what-we-know-so-far\" target=\"_blank\">44 regionally specific blueprints<\/a> for improving health and social care services and finances for the coming five years \u2013 are taking shape, and final versions are expected to be submitted in the coming weeks. With this in mind, the Nuffield Trust and the Healthcare Financial Management Association recently held a workshop for STP leaders that was designed to help those involved in STP \u2018footprints\u2019 to develop their plans. We have drawn together insights from the workshop, from members of our learning networks for hospital CEOs and CCG accountable officers, and from a series of conversations with other STP leads. We have also reviewed a number of draft plans.<\/p>\n<hr \/>\n<p><strong>BMA announce details of prolonged industrial action &#8211; NHS Employers<\/strong><br \/>\nOn 31 August the British Medical Association (BMA) Council confirmed it supported a recommendation for <a href=\"http:\/\/www.nhsemployers.org\/news\/2016\/09\/breaking-bma-announce-details-of-prolonged-industrial-action\" target=\"_blank\">industrial action<\/a> from the BMA Junior Doctors Committee.\u00a0 Industrial action will take place in the form of a full withdrawal of labour for five days, between the hours of 8am and 5pm during the following periods:<\/p>\n<ul>\n<li>12 &#8211; 16 September<\/li>\n<li>5, 6 and 7 October (weekend covered) and then 10 &#8211; 11 October<\/li>\n<li>14 &#8211; 18 November<\/li>\n<li>5 &#8211; 9 December.<\/li>\n<\/ul>\n<p>Emergency cover will not be provided.<\/p>\n<hr \/>\n<blockquote><p><strong>Case study published by doctors at Southport and Ormskirk Hospital<\/strong><br \/>\n<a href=\"http:\/\/www.jfas.org\/article\/S1067-2516(16)00038-7\/pdf\" target=\"_blank\">An Unusual Case of Kirschner Wire Migration in the Foot<\/a>. Kong, Roderick; Mohamed, Mohamed; Toh, Eugene.<strong> Journal of Foot &amp; Ankle Surgery<\/strong>; 09\/01\/2016.<br \/>\n&#8220;We describe a rare case, never before published, of migration of a Kirschner wire used for fusion of the proximal interphalangeal in a lesser toe in a 67-year-old female into the calcaneum. This patient presented to the orthopedic clinic 6 months after elective surgery complaining of pain in her right foot. Radiographs of the foot revealed that the Kirschner wire had migrated toward the calcaneum. Richards B, Riley J, Saithna A.<br \/>\n&#8220;A high rate of suboptimal shoulder radiographs was identified during a service evaluation exercise in our orthopaedic outpatient clinics. Inadequate radiographs require a return to the radiology department for further imaging, a resultant increased workload, delays in the clinic, increased radiation for patients, and inconvenience and decreased patient satisfaction. Furthermore, if a sub-optimal radiograph is accepted there is concern that diagnoses may be missed. This data gives trust-level insights about real-world chemotherapy treatment of patients in the NHS in England which was previously only available from clinical trials. The report shows that age, general wellbeing and other factors can affect 30-day mortality following chemotherapy for breast and lung cancer.
