This report outlines an organising framework and tested methods that local health and social care leaders can use to improve whole system flow. It draws on case studies and other examples of work from across the UK and internationally and describes steps that policymakers and regulators at a national level should take to create an environment that is conducive to change at this scale.<\/p>\n<hr \/>\n<p><strong>Medical professionalism matters: report and recommendations &#8211;\u00a0General Medical Council<\/strong><br \/>\nThis <a href=\"https:\/\/www.gooddoctors.org.uk\/medical-professionalism-matters-report\/\" target=\"_blank\">report <\/a>is the culmination of 18 months of events and conversations with the medical profession, from GPs, consultants and trainees who work on the ground, to royal colleges, training providers and employers. Among the most frequently raised issues were the lack of time and support to make a reality of reflective practice \u2013 at every stage of a doctor\u2019s career. Doctors also cited problems around professional isolation, fragmentation of care for patients and poor communication.<\/p>\n<hr \/>\n<p><strong>Lessons in leadership from women clinical commissioners: practical guidance to support the next generation of women clinical leaders &#8211;\u00a0NHS Clinical Commissioners<\/strong><br \/>\nThis <a href=\"http:\/\/445oon4dhpii7gjvs2jih81q.wpengine.netdna-cdn.com\/wp-content\/uploads\/2016\/12\/Lessons-in-leadership-from-women-clinical-commissioners_07122016.pdf\" target=\"_blank\">publication <\/a>aims to provide practical advice based on the experience of women CCG leaders at the top of their careers. It contains examples from women clinical commissioning leaders on the skills, values, and behaviours that helped them to succeed in their own roles. It also includes advice on supporting future women clinical commissioning leaders, including actively encouraging women colleagues to apply for leadership roles and making sure that wording in job adverts attracts rather than discourages female applicants.<\/p>\n<hr \/>\n<p><span lang=\"EN-US\"><strong>Medicines optimisation dashboard<br \/>\n<\/strong><\/span>The NHS Prescription Service has relaunched the <a href=\"http:\/\/www.nhsbsa.nhs.uk\/PrescriptionServices\/5891.aspx\">Medicines Optimisation Dashboard<strong>.<\/strong><\/a> The November 2016 dashboard is now available via a new platform based on NHS RightCare\u2019s InstantAtlas.\u00a0 The dashboard helps support NHS organisations in highlighting variation and facilitates discussion on how they compare with others across a range of comparators.<\/p>\n<hr \/>\n<p><strong><span lang=\"EN-US\">Morale of the medical workforce<br \/>\n<\/span><\/strong><span lang=\"EN-US\">The Royal College of Physicians has published <a href=\"https:\/\/www.rcplondon.ac.uk\/guidelines-policy\/keeping-medicine-brilliant\">Keeping medicine brilliant: improving working conditions in the acute setting<\/a>. This report focuses on developing the evidence base to support new ways of assessing and improving doctors\u2019 morale.\u00a0 The report also explores the issue of recruitment to the rank of medical registrar, and highlights that the perception of on-call roles as being extremely stressful and a significant deterrent to recruitment.<\/span><\/p>\n<hr \/>\n<p><strong>End of life care for infants, children and young people with life-limiting conditions: planning and management &#8211;\u00a0National Institute for Health and Care Excellence (NICE)<\/strong><br \/>\nThis <a href=\"https:\/\/www.nice.org.uk\/guidance\/ng61\" target=\"_blank\">guideline <\/a>covers the planning and management of end of life and palliative care in for infants, children and young people (aged 0\u201317 years) with life-limiting conditions. It aims to involve children, young people and their families in decisions about their care, and improve the support that is available to them throughout their lives.<\/p>\n<hr \/>\n<p><strong>Temporarily suspending patient registration commissioner guidance<br \/>\n<\/strong>NHS England has published <a href=\"https:\/\/www.england.nhs.uk\/wp-content\/uploads\/2016\/12\/suspend-pat-reg-respns-guid.pdf\">Commissioner guidelines for responding to requests from practices to temporarily suspend patient registration<\/a> (pdf). These guidelines have been published to assist commissioners in responding to general practices wanting to suspend patient registration on a temporary basis. It recognises the duty on commissioners to secure services for patients as well as the pressure on practices in providing services.<\/p>\n<hr \/>\n<p><strong><span lang=\"EN-US\">NICE shared learning<\/span><\/strong><\/p>\n<ul>\n<li><a href=\"https:\/\/www.nice.org.uk\/sharedlearning\/hydration-matters-a-quality-improvement-project-for-improving-intravenous-fluid-prescribing-practices-and-documentation-in-line-with-nice-guidance-cg174\">Hydration Matters<\/a> &#8211; a Quality Improvement Project for Improving Intravenous fluid prescribing practices and documentation in line with NICE guidance (CG174)<\/li>\n<li><a href=\"https:\/\/www.nice.org.uk\/sharedlearning\/adoption-of-urolift-procedure-an-ambulatory-pathway-for-patients-suffering-from-lower-urinary-tract-symptoms-of-benign-prostatic-hyperplasia\">Adoption of UroLift procedure<\/a> &#8211; an ambulatory pathway for patients suffering from Lower Urinary Tract Symptoms of Benign Prostatic Hyperplasia<\/li>\n<li><a href=\"https:\/\/www.nice.org.uk\/savingsandproductivityandlocalpracticeresource?ci=http%3A%2F%2Farms.evidence.nhs.uk%2Fresources%2Fqipp%2F1057369%2Fattachment%3Fniceorg%3Dtrue\">Improving and maintaining medicines reconciliation on admission<\/a>\u00a0&#8211; a project designed to ensure medicines reconciliation was carried out within 24 hours of admission, to help optimise prescribing and reduce medication errors<\/li>\n<\/ul>\n<hr \/>\n<p><strong>C<\/strong><strong>urrent systems missing new hospital infections, APPG report warns &#8211; NHE\u00a0<\/strong><br \/>\nNew healthcare-acquired infections (HCAIs) are <a href=\"http:\/\/www.nationalhealthexecutive.com\/Health-Care-News\/current-systems-missing-new-hospital-infections-appg-report-warns\" target=\"_blank\">being missed by current surveillance systems,<\/a> the All-Party Parliamentary Group (APPG) for Patient Safety warned in its final report. According to the report, \u201ca number of increasingly concerning\u201d HCAIs, including CPE, fall outside the scope of routine surveillance and monitoring&#8230;<\/p>\n<hr \/>\n<p><span lang=\"EN-US\"><strong>Ambitions for palliative and end of life care hub<br \/>\n<\/strong><\/span>NHS England has established the <a href=\"https:\/\/www.england.nhs.uk\/2016\/12\/nicola-spencer\/\">Ambitions for Palliative and End of Life Care website<\/a> which is a national Knowledge Hub for providing health professionals with a \u2018one stop shop\u2019 of palliative and EoLC information. These standards describe good practice and good systems of care for reporting, learning sharing, taking action and review of incidents as part of a patient safety culture.
