{"id":6086,"date":"2017-01-18T11:38:33","date_gmt":"2017-01-18T11:38:33","guid":{"rendered":"http:\/\/www.lihnnhs.info\/Sando\/?p=6086"},"modified":"2017-01-18T11:44:10","modified_gmt":"2017-01-18T11:44:10","slug":"knowledge-lert-for-wednesday-18th-january","status":"publish","type":"post","link":"https:\/\/www.lihnnhs.info\/Sando\/2017\/01\/18\/knowledge-lert-for-wednesday-18th-january\/","title":{"rendered":"Knowledge @lert for Wednesday 18th January"},"content":{"rendered":"<hr \/>\n<p><strong>Support overdue: women\u2019s experiences of maternity services &#8211;\u00a0NCT<\/strong><br \/>\nThis <a href=\"https:\/\/www.nct.org.uk\/sites\/default\/files\/related_documents\/Support_Overdue_2017.pdf\" target=\"_blank\">report <\/a>is based on the NCT&#8217;s second survey of women\u2019s experiences of their maternity care carried out with the WI. Women are generally positive about the maternity care they receive and many praised the kindness and professionalism of the staff who cared for them. \u00a0However, some findings gave cause for concern. For example, the survey asked about \u2018red-flag\u2019 events \u2013 indicators of dangerously low staffing levels, such as a women not receiving one-to-one care during established labour \u2013 and found that half of all women surveyed had experienced a red flag event. Postnatal care remains an area of concern with no change in the percentage of women \u2013 roughly one in five \u2013 telling us they were not able to see a midwife as much as they needed post-birth.<\/p>\n<hr \/>\n<p><strong>Quality gaps identified through mortality review &#8211; BMJ Quality &amp; Safety current issue<\/strong><br \/>\nHospital mortality rate is a common measure of healthcare quality. Morbidity and mortality meetings are common but <a href=\"http:\/\/qualitysafety.bmj.com\/content\/26\/2\/141.short?rss=1\" target=\"_blank\">there are few reports of hospital-wide mortality-review processes to provide understanding of quality-of-care problems associated with patient deaths. <\/a>A nurse and a physician independently reviewed every death that occurred at our multisite teaching institution over a 3-month period. Deaths judged by either reviewer to be unanticipated or to have any opportunity for improvement were reviewed by a multidisciplinary committee. We report characteristics of patients with unanticipated death or opportunity for improved care and summarise the opportunities for improved care.<\/p>\n<hr \/>\n<p><strong>International recommendations for national patient safety incident reporting systems: an expert Delphi consensus-building process &#8211; BMJ Quality &amp; Safety current issue<\/strong><br \/>\nPatient safety incident reporting systems (PSRS) have been established for over a decade, but uncertainty remains regarding the role that they can and ought to play in quantifying healthcare-related harm and improving care. We obtained reasonable consensus among experts on aims and specifications of PSRS. <a href=\"http:\/\/qualitysafety.bmj.com\/content\/26\/2\/150.short?rss=1\" target=\"_blank\">This information <\/a>can be used to reflect on existing and future PSRS, and their role within the wider patient safety landscape. The role of PSRS as instruments for learning needs to be elaborated and developed further internationally.<\/p>\n<hr \/>\n<p><strong>BJGP Open: primary health care journal launched<br \/>\n<\/strong>The Royal College of General Practitioners has launched <a href=\"http:\/\/bjgpopen.org\/\">BJGP Open<\/a>. This open access journal aims to serve the primary healthcare community, showcasing original research from around the world.
