{"id":9979,"date":"2017-12-11T14:50:44","date_gmt":"2017-12-11T14:50:44","guid":{"rendered":"http:\/\/www.lihnnhs.info\/Sando\/?p=9979"},"modified":"2017-12-11T14:52:50","modified_gmt":"2017-12-11T14:52:50","slug":"knowledge-lert-for-monday-11th-december","status":"publish","type":"post","link":"https:\/\/www.lihnnhs.info\/Sando\/2017\/12\/11\/knowledge-lert-for-monday-11th-december\/","title":{"rendered":"Knowledge @lert for Monday 11th December"},"content":{"rendered":"<hr \/>\n<p><strong>Dementia assessment and improvement framework: Framework\/compulsory guidance &#8211;\u00a0NHS Improvement<\/strong><br \/>\nAn <a href=\"https:\/\/improvement.nhs.uk\/resources\/dementia-assessment-and-improvement-framework\/\" target=\"_blank\" rel=\"noopener\">evidence-based framework<\/a> to support and enable <em><strong>directors of nursing and medical directors<\/strong><\/em> to achieve \u2018outstanding\u2019 care standards for those living with dementia during their stay in hospital.<\/p>\n<hr \/>\n<p><strong>Section 136 of the Mental Health Act<\/strong><br \/>\nThe Royal College of Emergency Medicine is reminding emergency departments of changes to section 136 of the Mental Health Act which take effect from today (11th December 2017).\u00a0 <a href=\"http:\/\/www.rcem.ac.uk\/RCEM\/News\/News_2017\/Important_Changes_to_section_136_of_the_Mental_Health_Act.aspx\">A brief guide to Section 136 for Emergency Departments<\/a> outlines the process to be followed when police bring a person to the department under a section 136.<\/p>\n<hr \/>\n<p><strong>Patient discharge from hospital<\/strong><br \/>\nNHS Improvement has published <a href=\"https:\/\/improvement.nhs.uk\/resources\/guide-developing-criteria-led-discharge\/\">A brief guide to developing criteria-led discharge<\/a>. This guide includes the rationale for developing criteria-led discharge, an outline of the two main approaches, underpinning principles and a review of related studies. <em>\u00a0<\/em><\/p>\n<hr \/>\n<p><strong>Flow of patients between the NHS and social\u00a0care &#8211; local performance metrics &#8211;\u00a0Department of Health<\/strong><br \/>\nA new <a href=\"https:\/\/www.gov.uk\/government\/publications\/local-area-performance-metrics-and-ambitions\" target=\"_blank\" rel=\"noopener\">dashboard <\/a>which allows local areas of assess the flow of patients across the boundary between the NHS and social care.<br \/>\nIt provides a set of six measures indicating how health and social care partners in every Local Authority area in England are performing at the interface between health and social care. Included in the dashboard is a breakdown of delayed days attributable to social care per 100,000 of the population and the equivalent for NHS-attributable delays.<br \/>\nThe metrics are:<\/p>\n<ul>\n<li>Emergency Admissions (65+) per 100,000 65+ population<\/li>\n<li>90th percentile of length of stay for emergency admissions (65+)<\/li>\n<li>TOTAL Delayed Days per day per 100,000 18+ population<\/li>\n<li>Proportion of older people (65 and over) who were still at home 91 days after discharge from hospital into reablement\/ rehabilitation services<\/li>\n<li>Proportion of older people (65 and over) who are discharged from hospital who receive reablement\/ rehabilitation services<\/li>\n<li>Proportion of discharges (following emergency admissions) which occur at the weekend<\/li>\n<\/ul>\n<hr \/>\n<p><strong>Avoiding admissions in neonatal units<\/strong><br \/>\nHealth Education England has released an e-learning programme to help healthcare professionals <a href=\"https:\/\/www.e-lfh.org.uk\/programmes\/avoiding-term-admissions-into-neonatal-units\/\">avoid term admissions into neonatal units<\/a>.
