Combating Loneliness.
Southport and Ormskirk Hospital NHS Trust patient experience strategy ‘Developing the Experience of Care’ centres around 8 co-produced pledges, one of which focuses on discharge and facilitating better links into Community Support. A project to combat loneliness was led by a Trust patient representative who was passionate about this subject and was able to work collaboratively with local third sector organisations. The result of this work produced an information booklet which supports relieving loneliness by including information on a number of local organisations that patients, families and carers can contact for support. It was felt that patients are offered a large amount of information at discharge, which at times is often overwhelming. Therefore it was thought that an A6 pocket sized booklet which included information from multiple organisations would be more ‘user friendly’.
Emergency care
NHS Improvement has published the following resources relating to emergency care:
- Five principles of ambulance flow – good practice in managing urgent care presentations to improve flow across the ambulance–emergency department interface.
- Hospital ambulance liaison officers (HALOs) – aims to help manage the hospital–ambulance interface and release ambulances quicker to respond to the next emergency
- Fit2Sit improvement tool – provides guidance on the principles of Fit2Sit
- The Emergency Care Intensive Support Team (ECIST) ambulance offer – outlines the core ambulance offer from ECIST, including resources and contact details
- Rapid improvement guide to system challenge events – provides a brief guide to running a system challenge event.
Work-based learning / Thriving at work
NHS Employers has collated tools and resources relating to the following:
- Building a culture of work-based learning – aims to support employers enable a culture of work-based learning. Topics range from senior leadership and management buy-in, to infrastructure, workforce planning and robust relationships with education providers.
- Thriving at Work standards – aims to support the implementation of standards to better support the mental health of all people currently in employment including those with mental health problems or poor wellbeing.
Older people’s programme evaluation
RAND Europe has published Southwark and Lambeth Integrated Care: evaluation of the older people’s programme final report. The study aims to estimate the impact on secondary care utilisation and costs of the Older People’s Programme; a multi-faceted primary care intervention, including Holistic Assessments and Integrated Care Management packages for older people identified as being at higher risk of avoidable hospitalisation.
NHS financial sustainability
The Public Accounts Committee has published NHS Financial sustainability: progress review. This review finds that the overall financial picture masks worrying disparities in performance of individual trusts and CCGs; staff shortages are a major obstacle to NHS financial viability and delivery of its Long Term Plan; and there is still no clarity on future funding for adult social care, capital, public health and education.
Fracture Liaison Service
The Royal College of Physicians has published Fracture Liaison Service Database Commissioner’s report 2019. This report shows that there are significantly fewer fracture liaison services (FLSs) available for older patients in some parts of England and Wales than others, with 54,000 preventable life-altering fractures estimated to occur over the course of the next 5 years as a result. The RCP has written to NHS CCGs calling for them to take action to ensure FLSs are available in their local areas.
Value of community health services
The Healthcare Financial Management Association has published The value of community services: comparison with acute settings. This briefing is the first of a series looking at how services delivered in the community add value to both the patient and the wider health and care economy. This report focuses on the value of delivering services in the community that were traditionally provided in an acute hospital setting.
