End of Life Care

When all efforts to improve a patients condition prove futile then we as a team with the family move to supporting a dignified end to the patient’s life. Patients will be kept pain and symptom free and they and their loved ones will continue to be supported by the intensive care team. We will also involve the Transform team.

This team provides education and support for clinical staff caring for people in the final months and days of life and supports patients and their families with Advance Care Planning discussions and provides emotional support for families of those thought likely to be dying where there may be no special palliative care needs identified.  https://www.queenscourt.org.uk/index.php/services/transform-team.html

It is important to us as a team that we recognise and support our patients spiritual needs  throughout their stay and at the end of life. We will involve the family and loved ones with planning and incorporate their religious and spiritual needs in this. Please see information on our Chaplaincy Team
