Type 2 Diabetes: Cardiovascular and Related Complications and Evidence-Based Complementary Treatments

Approximately 29 million Americans are diagnosed with Type 2 diabetes annually. Of that number, only about 36 percent (10.44 million diabetes sufferers) achieve satisfactory medical outcomes and would need additional help—rarely available—to reliably control their glucose levels. Contrary to popular belief, although anti-diabetic medications can lower sugar levels, nevertheless they have a poor performance track record because inflammation in the blood vessels persists.

This book details recent scientific findings that cardiovascular, kidney, vision, peripheral nervous system, and other body damage caused by chronic high levels of blood sugar (hyperglycemia) in Type 2 diabetes is actually due to excessive generation of unopposed free radicals and reactive oxygen species (ROS). These, in turn, cause chronic systemic inflammation and dysfunction of the endothelial lining of the arterial blood vessels, jeopardizing the formation of the protective molecule nitric oxide (NO), thus severely impairing the blood supply to every organ and tissue in the body. This book also catalogues the evidence that chronic hyperglycemia causes profound and often irreversible damage—even long before Type 2 diabetes has been diagnosed. In addition, because conventional prescription treatments are, unfortunately, often inadequate, the book details evidence-based complementary means of blood sugar control.

Poor quality dietary habits are one of the most pressing public health concerns of our time. As a society, we are faced with the paradox of malnutrition and overconsumption existing side-by-side. Many people in our communities deal with the stark reality of food insecurity coupled with a reliance on inexpensive, nutrient-poor calories that contribute to the nationwide prevalence of obesity, type 2 diabetes, heart disease, and other chronic conditions. As a resource for both students and practitioners, Public Health and Community Nutrition provides an overview of how social determinants of health and socioeconomic factors that influence a population’s or an individual’s well-being¾contribute to the existence of health disparities in the United States. Now more than ever, diet and health experts are needed to address these 21st-century public health challenges that require specific professional competencies related to nutritional assessment, knowledge of food assistance and support options, and nutrition education skills that are appropriate for targeted audiences.

Integrating care across disciplines and organisations around the needs of the person with diabetes has been proposed as an approach that could improve care while reducing cost- but has it and can it? Integrated Diabetes Care- A Multidisciplinary Approach collates evidence of worldwide approaches to both horizontal integration (across disciplines) and vertical integration (across organizations) in diabetes care and describe what was done, what worked and what appeared to be the barriers to achieving the goals of the programmes. Evidence is sought from groups who have developed different approaches to integrating diabetes care in different health systems (eg insurance vs tax payer funded, single vs multiple organization, published vs unpublished). A final chapter brings the evidence together for a final discussion about what seems to work and what does not.

Male weight problems are a serious public health issue and can lead to hypertension, hyperlipidaemia, diabetes and cancer. If current trends continue, the prospect of the majority of men becoming overweight is a very real one, and urgent action is imperative. The middle-aged spread that most men ‘expect’ to develop as they enter into their thirties and forties is now occurring much earlier with boys and young men developing life limiting weight problems. This multidisciplinary guide provides a gender sensitive approach to weight issues. Men need to be targeted specifically and in a male-focused manner, in order to overcome the multitude of contributing factors in their weight gain; social influences, dietary restrictions, education, cultural expectations, psychological considerations and exercise regime. This book takes a balanced approach, offering practical guidance as well as evidence-based research, academic perspectives and personal experiences. The advice is easy to implement and has been proven in real-life settings. All healthcare professionals, nutritionists and dieticians will find the assistance invaluable. It is also highly recommended for psychologists, counsellors and therapists, particularly those working with men. Healthcare policy makers and shapers too, will find much of interest. ‘A definitive and seminal book that will change the way male weight problems are tackled in the UK and beyond.’

Hypertension is a highly treatable risk factor for cardiovascular disease, yet it remains underdiagnosed, undertreated and, in many cases, poorly controlled. Sometimes dubbed ‘the silent killer’, hypertension rarely produces symptoms until it manifests as a heart attack or stroke. Early identification of the condition by routine blood pressure measurement and appropriate ongoing management are therefore vital. In this updated fifth edition of Fast Facts: Hypertension, two authors of international renown provide a practical evidence-based approach to the diagnosis and management of the condition. The book provides a thorough overview of hypertension, covering: – Accurate measurement of blood pressure and problems such as ‘white-coat’ hypertension – Routine assessments for people with high blood pressure – Consideration of other modifiable risk factors – smoking, high cholesterol levels, etc. – Non-pharmacological measures that doctors can advise on – reduce salt, lose weight, increase physical activity, etc. – Up-to-date trial results on all the pharmacological options – Special considerations for the treatment of children, pregnant women, the elderly and patients with diabetes. Primary care physicians have a leading role to play in screening for hypertension and ensuring that patients receive the most appropriate treatment. Fast Facts: Hypertension provides a concise readable refresher on this common and potentially devastating condition.

Published: 01 January, 2016
eISBN-13: 9783319438276

This book addresses the unique healthcare needs of adults with chronic childhood illnesses. It presents a model of primary and secondary prevention for emerging adulthood-primary prevention in which all young adults are screened for high-risk behaviors and health needs and secondary prevention in which young adults with chronic childhood conditions are optimized through coordinated care, connections to community resources and social/family support. This book is organized in five parts. Part I provides a detailed overview of the health care transition from pediatrics to adult medicine from both a policy and practice perspective. In Part II, the concept of emerging adulthood as a developmental period is explored and strategies for providing improved comprehensive care for this age group are discussed. Part III reviews specific chronic childhood conditions, such as attention-deficit/hyperactivity disorder, autism, cystic fibrosis, and diabetes mellitus, and offers clinical cases and summary reports that can be used as a quick guides to each condition. In Part IV, additional clinical considerations that are not necessarily condition-specific but are highly relevant to the care of young adults with chronic childhood conditions are examined. Part V describes the socio-legal issues involved in caring for this population. Care of Adults with Chronic Childhood Conditions provides primary care providers with a new framework for the care of young adults and identifies opportunities to influence patient health outcomes over a life trajectory.
